Heart Failure
A diagnosis of advanced heart failure, also known as congestive heart failure or CHF, indicates heart disease that has progressed to a point where traditional therapies and symptom management are no longer working. Over time, the heart becomes enlarged, and the tissue becomes damaged. Heart failure is most common in people over age 65 and is a leading cause of hospitalization for this age group.
Patients are typically referred to our program by their cardiologist or primary care physician.
