ElderLink’s case management services

We provide affordable case management to older adults in Fairfax County & the Cities of Falls Church & Fairfax. Our care managers work with you and your family to find the best plan to ensure safe and affordable independence. Our services include:

  • an in-home assessment to determine what type of care is needed;
  • development of a comprehensive care plan;
  • assistance in finding and obtaining services;
  • monitoring to ensure appropriate care.

ElderLink’s Case Management Services are available at an affordable fee for service and on a sliding fee scale. Co-pay rates are dependent on the client’s functional (physical/cognitive needs) and/or financial status.

ElderLink’s free caregiver support programs

Need an in-depth conversation with a social worker about the caregiving needs of a loved one? Free one-hour consultations are available through ElderLink. Call 703-324-5374, TTY 711, to request a Caregiver Consultation.
 

ElderLink hosts a Virtual Support Call for Caregivers on the 2nd Tuesday of each month from 7:00-8:00PM. Focuses on a different topic each month and provides a convenient, safe space for caregivers to learn, share, and connect with one another. Registration is required. This unique training opportunity which allows participants to “see through the eyes” of older adults is available through a new virtual training series called Exploring the Dimensions of Aging through Virtual Reality. The training series was developed by Embodied Labs and simulates what it is like to be an older adult encountering various challenges in everyday situations. 75-min facilitated virtual sessions focus on how social isolation impacts an older adult, the early signs and progression of Lewy Body Dementia and Parkinson’s Disease, how to navigate caregiver burnout, and much more. The Volunteer Respite Program is designed to give caregivers of older adults a temporary break from their caregiving duties. Trained volunteers provide up to 6 hours per month of companionship and supervision, no personal care provided. There are no income requirements for this program and no fee for service. Eligibility criteria include an assessment of the older adult’s physical/cognitive needs.

ElderLink’s Sliding Fee Scale programs

The following services are available on a sliding-scale basis taking into consideration medical and other care expenses:

ElderLink Bathing Program
The ElderLink Bathing Program offers in-home personal care services including up to 2 baths per week for eligible clients. This service includes a full assessment and ongoing case management. Co-pay rates are dependent on the client’s functional status (physical/cognitive need) and income level.

ElderLink Respite Program
The ElderLink Respite Program is designed to give full-time, family caregivers a temporary break from caregiving duties. This program offers up to 6 hours per week of in-home personal care services, companionship, and supervision for eligible clients. This service includes a full assessment and ongoing case management. Co-pay rates are dependent on the client’s functional status (physical/cognitive need) and income level.
